Chronic Disease Self Management Program (CDSMP)
The Chronic Disease Self-Management Program (CDSMP) is a volunteer-led education program offered in communities throughout British Columbia. Participants are adults experiencing chronic health conditions (e.g. hypertension, arthritis, heart disease, stroke, lung disease, diabetes), their family members, friends and caregivers. The program provides information and teaches practical skills on managing chronic health problems. Most importantly, the CDSMP gives people the confidence and motivation they need to manage the daily challenges of living with a chronic health condition.
The program is currently being implemented throughout British Columbia by the University of Victoria - Centre on Aging with assistance from the BC Ministry of Health. The book included with this program is “Living a Healthy Life with Chronic Conditions” by Dr. Kate Lorig, Dr. Halsted Holman, Dr. David Sobel et al.
